Residency Application Requirements

Qualifications

  • Doctor of Pharmacy Degree from an ACPE-accredited school of pharmacy
  • Eligible for licensure in the state of West Virginia
  • PGY2 Applicants must have successfully completed a PGY1 Residency in Pharmacy Practice
  • Residency requires a specific set of minimum physical, mental, emotional and social abilities necessary to be successful. Residents must possess all of the abilities listed in the five categories outlined in this policy (See CAMC Staff Handbook).

Application procedure

All application materials must be submitted via PhORCAS

  • Letter of intent
  • Copy of transcripts
  • Three letters of recommendation
  • Curriculum vitae
  • Passport-style photograph (for identification purposes only during application review and interview process)
