Residency Application Requirements
Qualifications
- Doctor of Pharmacy Degree from an ACPE-accredited school of pharmacy
- Eligible for licensure in the state of West Virginia
- PGY2 Applicants have successfully completed or are completing an ASHP accredited or candidate-status PGY1 Pharmacy Residency
- Residency requires a specific set of minimum physical, mental, emotional and social abilities necessary to be successful. Residents must possess all of the abilities listed in the five categories outlined in this policy (See Staff Handbook).
